You’ve been selected to complete federal verification. Verification is the process of confirming the information reported on your Free Application for Federal Student Aid (FAFSA). As part of that process, we will verify information regarding your household size. To provide this information, complete the form on this page based on the campus you are attending.

Answer questions to help you complete your worksheet. Once you're done, you'll receive an email with a filled-in form. Just print, add your signature (and a parent's), and submit.

Once you’ve completed the form, please submit it to the Office of Student Financial Services at the address listed on the form. Please allow up to 15 business days for your documentation to be reviewed. Additional documentation may be requested, and any form that is incomplete or provides discrepant information will require follow-up and delay processing. If we are unable to complete verification, we may be required to cancel any existing financial aid awards you might have received.
Instructions for filling out the form
If your legal parents are not married to each other:
- Answer the questions based on the parent who will provide more than 50 percent of your support from July 1, 2022 through June 30, 2023.
- If the parent providing the support has married or remarried, stepparent information is required.
List all people that should be included in your parent’s household, starting with yourself. You should include your parent, his or her spouse (if married), and any children or dependents living in the home if your parent will provide more than half of their support from July 1, 2022 through June 30, 2023.
If your parent pays child support for a child, do not include that child in your household size.
Sign the worksheet (along with a parent). Electronic signatures are not accepted. Make sure to provide your university ID on the worksheet to ensure there is no delay of processing. Please note that in some cases additional documentation may be requested.
