Do you need to complete a Release of Information form?

If any of your external awards require the Office of Student Financial Services to submit information to the donor (e.g., cost of attendance, federal aid received) in order for you to receive the award, please complete the authorization release form.  This is only for use with your scholarship provider in order for us to provide the necessary information to your provider.
