Once you’ve completed the form, please submit it to the Office of Student Financial Services at the address listed on the form. Please allow up to 15 business days for your documentation to be reviewed.
Additional documentation may be requested, and any form that is incomplete or provides discrepant information will require follow-up and delay processing. If we are unable to complete verification, we may be required to cancel any existing financial aid awards you might have received.
Instructions for filling out the form
List all people that should be included in your household, starting with yourself and your spouse (if applicable). You may list any children or dependents living in your home if you will provide more than half of their support from July 1, 2024 through June 30, 2025.
Sign the worksheet. Electronic signatures are not accepted. Make sure to provide your university ID on the worksheet to ensure there is no delay of processing. Please note that in some cases additional documentation may be requested.
